What We Do for Families

Wraparound Care
Our approach is built on a family-centered, community-based process that puts your family in the driver’s seat. A dedicated Care Manager works with you to identify your family’s unique strengths and build a customized plan of care.

Child and Family Team Meetings
Your Care Manager will help bring together your team to develop and carry out an Individual Service Plan (ISP) tailored to your child’s needs and your family’s goals.

Nurtured Heart
The Nurtured Heart Approach uses strategies that help youth in developing their self-regulation, focusing on transforming the way youth perceive themselves, their caregivers, and the world around them.

Connection to Resources
We connect families with a wide network of service providers, including specialized programs, therapy providers, educational supports, and more.

Crisis Planning
Your Child & Family Team will develop a clear crisis plan, so that if a difficult moment arises, everyone knows exactly what to do and who to call.

Peer Support
Through our partnership with the Family Support Organization (FSO) of Middlesex County, families are connected with a Parent Partner with lived experience who provides free, compassionate support.
